OSCE QUESTIONS :

- The patient was diagnosed hypertensive 1.5 years ago but was not compliant to the medication. What were the chances that her hypertension would have decreased if she takes medication?? as she has central obesity and sarcopenia

- Was the post nasal surgery required 1.5 years ago with the complaints of patient ?

- Her RFT is normal. What is the first marker to raise in Renal failure?

- The clinical presentation of the patient was like metabolic syndrome, but Her RBS was normal, Is there any chance that her pre and post prandial glucose to raise?

- The patient is obese what advice regarding the diet and daily routine is given . What is done to know whether she is following your advice or not?

LEARNING POINTS :

- I got to know that Cystatin - C is the first marker to be elevated in the patients with renal failure whose RFT is completely normal

- The nasal decongestants like oxymetazoline, xylometazoline can also be a factor for her raised blood pressure

- This is the first case I have seen that HTN has lead to visual disturbances as she had grade 3 Hypertensive retinopathy.
